Help needed with a website "mail-to" form......

I'm building a website for a friend of mine, and she wants a form on the site where you fill in the blanks and click a "Submit" button, but I'm stuck. I found some help pages and free form generators, but nothing that I can get to work yet.

The link below is NOT the site I'm working on, but the form there is very similar to what I'm making......just so you can see what I'm talking about.

Does anyone have any tips on something like this? I'm using Dreamweaver to make the rest of the site.

is the server running php or asp or perl. you make the form and then u send it to a server side script witch will then drop it in your e-mail very easy to do just need to know more info about your webhost
