The last company I worked for used an Astaro firewall to do it. Astaro updates their blacklist daily and the firewall would download the latest one from them nightly.

It's a combination of meta info and web-crawler spiders (that process content & language).

This is also why certain words are censored from the board - to try and keep 'under the radar' as much as possible. It's not 100% effective, but it helps out a bit.
